Grundy County is ranked healthiest in the state and Monona County is the least healthy. The least-healthy county for health outcomes is Union, and Putnam rated second from last in the state.

Dutchess County ranked 14th for health outcomes and ninth for health factors an improvement from 17th in health outcomes past year.

The regional partnership of Brown, Nicollet, Waseca and Le Sueur counties provide small grants to schools and other organizations to build gardens or establish other features to benefit community health.

For the 2018 Rankings, Broome County improved its Length of Life ranking to 52 out of 62 counties.

Doctors in the US earn twice the average salary than their peers in 10 other high-income countries. "For example, while it's 11 percent overall, it's 53 percent of black children live in poverty". Ozaukee County topped the list at No. 1 for both its health outcomes and factors.

Census data supports the correlation: The median income in Rankin is $59,370, while in Holmes it's $20,800, the lowest in the state.

"No one should have less of an opportunity to be a healthy because of where they live", Marjorie Paloma, the foundation's program director said during a morning press conference announcing the latest report at the Statehouse in Trenton.

"We definitely look at this and see what areas we need to try harder on improvements", she said.

Public health officials are required to do comprehensive community assessments every five years and then develop a plan to address the top identified issues. "Quick solutions to the complexities of public health don't always fit desired solutions", he said.

"We're talking about very small increments of change from year to year and from county to county", Hildebrandt said.

The counties partner with Watonwan County as part of the Statewide Health Improvement Partnership, established across the state to create healthier communities.
